ENROLMENTS

Enrolments for the Dec 2011/Jan 2012 School Holiday Programmes are open NOW.  Please read the following information, then click on the appropriate link to download/print the correct enrolment form.

 

Enrolments may be made either in person at the preferred venue site Before & After Care, at our Bizzy Bodz office during the times outlined below, by fax to 277 0128, or by post to PO Box 38 742 Howick.  All fax enrolments MUST include credit card details.  All fees are to be paid upon enrolment in order to secure a place for your child.

REMUERA ENROLMENT FORM

 

 

All families must complete an enrolment form for each child attending the programme, each holiday.  This is to ensure all personal and contact details are kept up to date, to ensure the complete safety of your children. Each form must be signed by a parent or guardian before they can participate in the programme.

 

Any changes to personal details must be notified as soon as possible to the programme co-ordinator, as outlined in the enrolment form, to ensure the continued safety of all children.
